- Patient Responsibilities: Patients are responsible for providing accurate medical history information, following prescribed treatments and instructions, and disclosing any relevant information to the healthcare providers.
- Payment: Payment for services rendered is due at the time of service unless other arrangements have been made in advance. Shivayu Superspeciality Eye Hospital accepts various payment methods, and charges are subject to change without prior notice.
- Appointment Cancellation: Patients are requested to provide at least 24 hours' notice if they need to cancel or reschedule an appointment. Failure to do so may result in a cancellation fee.
- Limitation of Liability: Shivayu Eye Hospital shall not be liable for any direct, indirect, incidental, special, or consequential damages arising out of or in any way connected with the use of its services.
